Form G-32 Information

Prior to the Time of First Execution, the underwriter must submit a Form G-32 to EMMA. Information required by Form G-32 includes such issue-specific information as the:

• • Full issuer name and issue description
• • Dated date
• • Expected closing date
• • Date and Time of Formal Award
• • Time of First Execution of the issue
• • Existence of a continuing disclosure undertaking by the issuer

For municipal fund securities and issues that are ineligible for CUSIP number assignment, the issuer must also indicate its state of residence.

Form G-32 also requires security-specific information consisting of the:

• • Nine-digit CUSIP number
• • Principal amount at maturity
• • Initial offering price and the initial offer
